The Ghana Patient Safety Forum is the country's foremost convening on the quality and safety of care. Now in its maiden edition, the 2026 Forum brings together frontline clinicians, hospital leaders, policymakers, researchers, and patient advocates to move from national commitment to daily practice.
Anchored in the World Health Organization's Global Patient Safety Action Plan and Ghana's National Quality of Care Strategy, the FORUM THAT WILL IGNITE MEANINGFUL DISCUSSIONS. Every session is designed to produce tools, commitments, and networks that return with delegates to their facilities.

Three tracks, one goal
The 2026 programme is organised around patient safety themes. Each combines keynote insight, lived experience, and panel discussions so delegates leave with something they can use in their daily practice.
Human Factors Ergonomics
Designing systems, tools, and environments that work with — not against — human limitations. From medication labelling to theatre layout, reducing error by design rather than relying on vigilance alone.
Patient & Family Co-production
Patients and families as partners in safe care — not passive recipients. Voice in decisions, honest disclosure after harm, and building a just culture where lived experience shapes improvement.
Patient Safety through Effective Teamwork
How teams communicate, coordinate, and recover from error. Briefings, debriefs, speak-up culture, and shared mental models that turn a group of clinicians into a reliable safety system.
